> Pricing includes your medical review and/or consultation with a medical provider as well as medication/supply costs paid to the pharmacy through invoicing or store purchase
> Complete the forms in the link below to determine if you are a good candidate, you will be sent an email with instructions on how to book an appointment if one is desired after completion of the forms, as well as provided with a password to order medications
> If you are a cleared by one of our NPs, medications will be sent directly to you from one the FDA approved pharmacies we partner with or sent to your pharmacy depending on preference
> Once registered on PracticeQ (our HIPPA compliant charting system) you will be invited to a chat portal where we can be reached with general or dosing questions/concerns/check ins

Available post medical clearance with a provider, will be sent directly to you. These are vial sizes, not dosing. You will be sent an invoice for the requested amount once cleared, we will help guide you if needed! Supplies included.

Metformin XR
Extended release metformin can be sent directly to you from one our FDA approved pharmacies or ordered directly to your preferred pharmacy for pick up.
Zepbound Vials from the Eli Lilly Pharmacy
Medication orders will be sent monthly to the Eli Lilly pharmacy. There will be a flat fee for the order and medical evaluation and you will pay the pharmacy directly for the medication depending on the dose you take. Please note this is an out of pocket expense to the pharmacy, insurance will not apply. I would advise checking prices per vial at the link below.
